MEMBERSHIP POLICY
-Contract term is 6 months, please note that at the end of the initial 6 month term, the membership automatically renews, unless a proper termination request is submitted.
-Termination of Membership/Termination of Auto-Renew- You may terminate your membership/auto-renew by providing a written or verbal request. Termination requests need to be submitted at minimum 14 business days prior to next billing cycle/auto-renew date.
If terminating the contract/membership prior to the end of the current 6 month term, an early cancellation fee in the amount of $100 is due. Any unused membership credits will expire according to their original expiration date.
-Each facial credit expires (60 days) after the membership fee has been charged. When utilizing the rollover credit, Facial Suite permits the use of one rollover credit per month.
-Facial Suite continues to adhere to our Appointment Cancellation Policy and kindly ask that you provide us with a 24 hr cancellation notice, or the appointment will be considered a used facial credit. By adhering to our cancellation policy, any appointment properly cancelled can be rescheduled.
-Memberships, perks, benefits and Add-On services are transferable and non-refundable.
MEMBERSHIP CANCELLATION POLICY
This is a 6-month contract and there is a $100 early cancellation fee. Credits are
non-transferable. Credits roll over once. Your card will be charged every first of the month.
